Free home safety modifications — do you qualify?

New York State's Medicaid program funds home accessibility and safety modifications at no cost for eligible members through regional Social Care Networks (SCNs). Grab bars, ramps, widened doorways, bathroom modifications, mold remediation, air conditioners for certain health conditions, and more.

You may qualify if all of these are true

  • ✓You have New York Medicaid through a managed care plan (most members do).
  • ✓You're in a priority group — for example: living with a physical disability or serious health condition, pregnant or postpartum, frequent ER or hospital visits, a Health Home member, or a child with high-risk health needs.
  • ✓A screening shows an unmet housing need, and your health provider attests the modification is medically necessary.

Only your regional Social Care Network can determine eligibility — it starts with a free screening by a Social Care Navigator. HomeBriit performs the work for eligible members; we don't decide who qualifies.

  2. A navigator screens you and confirms eligibility for enhanced services.
